Terms of Agreement

1. I agree to abide by the U.S. Copyright Law and Fair Use guidelines:
2. The copyright law of the United States (Title 17, US Code) governs the making of photocopies or other reproductions of copyrighted material. Under certain conditions specified in the law, libraries and archives are authorized to furnish a photocopy or other reproduction. One of these specified conditions is that the requested photocopy or reproduction is not to be "used for any purpose other than private study, scholarship, or research." If a user makes a request for, or later uses, a photocopy or reproduction for purposes in excess of "fair use," that user may be liable for copyright infringement. This institution reserves the right to refuse to accept a copying order if, in its judgment, fulfillment of the order would involve violation of copyright law. (37 C.F.R. 201.14)
3. RUSH handling includes an additional charge of $10.00. I agree to pay any additional RUSH fee when it is incurred.
4. To CANCEL a request, I will immediately call 352.273.8447 with the request number. I understand that I may not be able to avoid fees and charges if the request has already been processed or filled.
5. I understand that I will be billed monthly for items which have been supplied. I agree to pay the Health Science Center Library within 30 days of the receipt of the invoice. I understand that failure to do so may result in suspension of this library service.
6. I understand that the Health Science Center Library will provide at least 30 days’ notice of changes in fees.
